Mounjaro® is a first-in-class, prescription medication that is designed to help with weight loss and manage type 2 diabetes. It is administered once weekly via self-injection. When combined with dietary and lifestyle changes, Mounjaro® can help you to achieve significant and sustained weight loss.
Mounjaro® is approved for use in adult patients with:
- A BMI (body mass index) of 30 kg/m2 or above (obese)
- A BMI between 27 and 30 kg/m2 (overweight) and a health problem that is related to weight, such as high blood pressure, high cholesterol, pre-diabetes or polycystic ovarian syndrome (PCOS)
Mounjaro® contains the active ingredient tirzepatide, which activates the receptors for 2 naturally occurring hormones – GLP-1 and GIP. These hormones decrease your appetite, reduce cravings and slow the emptying of your stomach making you feel fuller for longer. The result is that you should consume fewer calories than you are burning, which will in turn lead to weight loss.
If you have no other medical conditions, it is not a requirement to have blood tests prior to starting treatment. However, we would strongly recommend a routine set of bloods at your first consultation and after 6 months to help us monitor your overall health on your weight loss journey.
Results will vary from person to person, but in a recent clinical trial patients who were on the highest dose of Mounjaro® lost an average of 21% of their body weight by week 72. In other words, if you weighed 90 kg at the start of treatment, you might weigh 71 kg after 72 weeks.
As with any medication, you may experience side effects after starting treatment. Common ones include nausea, tummy pain, diarrhoea and constipation. Side effects are most likely when you start the medication or increase the dose. Mild side effects usually go away within a few days to a week as your body adjusts to the medication.
Mounjaro® may not be suitable for everyone. Reasons why this treatment might not be appropriate for you include if you have previously had pancreatitis, if there is a history of certain health problems that run in the family or if you are pregnant or planning a pregnancy. Our team will review your medical history in detail to ensure Mounjaro® is the right treatment for you before recommending this medication.
Yes, it is important to inform your GP if you start Mounjaro®, especially if you are on other medications or have existing health conditions. Your GP can ensure Mounjaro® will not interact with any other treatment you might be on and offer ongoing advice.
If you start treatment with Mounjaro®, you will be reviewed monthly to assess your progress, monitor any side effects, and make any necessary adjustments to your treatment plan. These reviews can be conducted either in person or virtually, depending on your preference.
Mounjaro® is extremely effective when combined with dietary and lifestyle changes, but results are not guaranteed. If you are not losing weight, this might be because the medication needs more time to work, the dose needs to be increased or because the medication is not right for you.
While you may lose some muscle when you lose weight, studies suggest that you will lose 3 times as much fat, so your overall body composition will improve. You should aim to eat plenty of protein and incorporate strength-based exercises into your normal routine to minimise muscle loss.
While alcohol does not directly interact with Mounjaro®, it can worsen common side effects like nausea, vomiting and stomach discomfort. Alcohol also contains additional calories, which will slow down your weight loss journey. Our advice would be to limit your alcohol consumption if taking Mounjaro®, but you do not have to stop drinking completely.
Mounjaro® may reduce the effectiveness of oral contraceptive pills, particularly if you experience side effects such as vomiting when starting treatment. If you are taking birth control pills, you should use an additional form of contraception (such as condoms) for at least 4 weeks after starting treatment and every time you increase the dose.
Mounjaro® has not been studied in pregnancy or breastfeeding and therefore cannot be recommended for use in these situations as its effect and safety profile are unknown. If you are planning a pregnancy in the future, you should aim to come off Mounjaro® at least 2 months before trying to conceive.

Injecting and storing Mounjaro®
Mounjaro® comes in a pre-filled, multi-dose, disposable device called a Kwikpen®. Each pen contains 4 fixed doses, which are self-administered at the same time every week.

Mounjaro® is available in 6 different strengths: 2.5 mg, 5 mg, 7.5 mg, 10 mg, 12.5 mg and 15 mg. When you begin your treatment journey, you will be started on the lowest dose (2.5 mg). This dose can be increased by 2.5 mg every 4 weeks until you reach the maximum dose of 15 mg once weekly.
Everyone will react slightly differently to Mounjaro®, and the standard dosing schedule may not be appropriate for you. Some people experience significant weight loss at lower doses or need to stay on lower doses for longer due to side effects.
The team at The Clinic Holland Park will work closely with you to create a bespoke dosing strategy to help achieve your weight loss goals in a healthy and sustainable manner.
If you forget to take your scheduled dose of Mounjaro®, take it as soon as possible within 4 days of the missed dose. If more than 4 days have passed, skip the missed dose and take your next dose on your usual day as scheduled. You may find it helpful to set an alarm on your phone to remind you to take the medication at the same time each week.
Unused pens should be kept in the ‘fridge prior to use. Once the first dose of your pen has been administered, it can be stored at room temperature – away from any heat sources and out of direct sunlight. If stored at temperatures above 30°C or under 2°C, it may be unsafe to use and should be discarded.
Once a pen device is finished, you should dispose this in a sharps disposal container or take it to your local pharmacy. Do not dispose of used pens in household waste.
Current evidence suggests that you are likely to regain weight if you stop this medication. However, some people might be successful in stopping the medication if they have achieved sustainable changes to diet and lifestyle during treatment. Others may be able to maintain a stable weight on a low maintenance dose of medication.
Starting Mounjaro®: What to expect in the first 3 months
Before starting: Preparation is key
A new medication is only one part of a successful weight loss journey. Starting Mounjaro® is a chance to reset your relationship with both food and exercise. Think about the following before starting treatment:
- Diet: you should aim to heat healthy, balanced meals and limit foods that are highly processed and high in sugar and fat. Prioritise protein to maintain lean muscle mass and fibre to prevent constipation. Try to get rid of any unhealthy foods in the house and think about changing your weekly food shop or meal delivery plan.
- Exercise: you should stay active and perform strength-based exercises to stay healthy and preserve muscle. Block out times in your diary for exercise and think about a gym membership and/or personal trainer to keep you accountable.
- Start date: you can administer your first injection at your clinic appointment or afterwards at a time of your choice. Pick a day and time and stick with it. Side effects tend to be worse just after the injection, so consider taking the medication at the weekend if you work during the week, and at night to decrease symptoms during the day.
- Side effects: these are common when you start treatment, but usually manageable. Over-the-counter medications like paracetamol, buscopan and Gaviscon are safe to use, and can help with symptoms of abdominal pain and reflux. Consider having these in the house before your first injection.
Week 1: A new week and a new you
The initial response to Mounjaro® can vary in the early stages. Some patients may notice a drop in weight within days (particularly if your diet was heavier in carbohydrates), while others will see more gradual changes. You are likely to notice a decrease in appetite in the first week of treatment.
We are aiming for sustainable and healthy weight loss, and the early stages of treatment are more about your body getting used to the medication and building healthy habits going forward.
Potential weight loss: 1% from baseline
Side effects: Nausea, diarrhoea and constipation are common – these should subside as the body adjusts, but may resurface with increases in dose
Week 2: Continued adjustment
By now, you should have had your second 2.5 mg dose of Mounjaro®. The effects of the medication may be more obvious, both in terms of appetite and weight.
At this stage, consistency with dietary changes and your exercise plan are vital for building sustainable habits for the future.
Potential weight loss: 1% per week from baseline
Side effects: these may continue but should improve as your body adapts to the medication – contact the team at the clinic or your GP if you are struggling.
Weeks 3-4: Smoother sailing ahead
As your body grows accustomed to Mounjaro®, weeks 3 and 4 often bring a reduction in side effects. The 2.5mg dose continues, but many find that hunger becomes easier to manage, and that you feel fuller for longer. Your body should now be operating at a more stable level, both in terms of medication and weight loss. Even though you are consuming fewer calories, it is crucial to maintain a balanced diet and take regular exercise.
Potential weight loss: up to 4% from baseline
Side effects: usually resolved at this stage
Weeks 5-8: First dose increase
The dose of Mounjaro® will typically increase to 5 mg in week 5, and this is when you are likely to see more consistent weight loss. Occasionally, a joint decision may be taken to continue with the 2.5 mg dose if you are already achieving significant weight loss or you are experiencing significant side effects. Your weight may fluctuate on a day-to-day basis, but the overall trend should be one of weight loss, and you may already have noticed a change in appearance and clothes size.
Potential weight loss: up to 5-6% from baseline
Side effects: likely to recur with the increase in dose, but usually resolve within a few days
Weeks 9-12: Reaching new milestones
The dose of Mounjaro® will usually go up to 7.5 mg at week 9. By now, you are likely to have noticed considerable changes to your weight, appearance and general sense of wellbeing. Changes to your diet should now be established and feel like second nature. Higher medication doses should offer stronger appetite suppression, making it easier to stick to your calorie targets.
Potential weight loss: up to 9-12% from baseline
Side effects: as with any dose increase, side effects may resurface but these should be temporary and manageable
Week 13 onwards: Steady progress
At week 13, the Mounjaro® dose is likely to go up to 10 mg. At this point, the overall health improvements should be noticeable to family, friends and colleagues. Changes to your routine – both in terms of food and exercise – should now be firmly established.
Potential weight loss: up to 13-15% from baseline
Side effects: again, likely to recur with any dose increase but typically temporary and manageable.
Long term results
Mounjaro® is a long-term medication that works best when combined with changes to diet and lifestyle. Clinical studies show that maximum weight loss is achieved after 72 weeks.
The maintenance medication doses used in the above study were 5 mg, 7.5 mg and 15 mg, but it is possible to stay on any dose that is working well for you.
